ABSTRACT

@#Objective     To investigate the safety of thoracic surgery for high-altitude patients in local medical center. Methods    We retrospectively collected 258 high-altitude patients who received thoracic surgery in West China Hospital, Sichuan University (plain medical center, 54 patients) and People's Hospital of Ganzi Tibetan Autonomous Prefecture (high-altitude medical center, 204 patients) from January 2013 to July 2019. There were 175 males and 83 females with an average age of 43.0±16.8 years. Perioperative indicators, postoperative complications and related risk factors of patients were analyzed. Results    The rate of minimally invasive surgery in the high-altitude medical center was statistically lower than that in the plain medical center (11.8% vs. 55.6%, P<0.001). The surgical proportions of tuberculous empyema (41.2% vs. 1.9%, P<0.001) and pulmonary hydatid (15.2% vs. 0.0%, P=0.002) in the high-altitude medical center were statistically higher than those in the plain medical center. There was no statistical difference in perioperative mortality (0.5% vs. 1.9%, P=0.379) or complication rate within 30 days after operation (7.4% vs. 11.1%, P=0.402) between the high-altitude center and the plain medical center. Univariate and multivariate analyses showed that body mass index≥ 25 kg/m2 (OR=8.647, P<0.001) and esophageal rupture/perforation were independent risk factors for the occurrence of postoperative complications (OR=15.720, P<0.001). Conclusion    Thoracic surgery in the high-altitude medical center is safe and feasible.

ABSTRACT

Surgery is an important method for the treatment of resectable esophageal cancer. With the continuous development of minimally invasive esophageal technology, video-assisted thoracoscopic esophagectomy (VATE) has demonstrated obvious advantages over conventional open surgery and has been widely accepted. However, there are still some esophageal cancer patients who cannot benefit from VATE. Inflatable video-assisted mediastinoscopic transhiatal esophagectomy(IVMTE) does not require one-lung ventilation, reduces postoperative complications, expands surgical indications, and brings surgical opportunities for patients with impaired lung function and thoracic lesions, which has become a new choice for the treatment of esophageal cancer. However, the limited field of surgical view and the tunneling surgical approach undoubtedly increase the difficulty of surgery, and how to clearly expose the anatomical structures and thoroughly dissect the lymph nodes has always been the key points and difficulties of surgery. The authors review relevant literatures to discuss the clinical progress and limitations of IVMTE.

ABSTRACT

Esophageal cancer is a common malignant tumor in China. For resectable ones, surgery is still the primary treatment. At present, the extent of lymph node dissection remains controversial. Extended lymphadenectomy makes metastatic lymph nodes more likely to be resected, which contributed to pathological staging and postoperative treatment. However,it may also increase the risk of postoperative complications and affect prognosis. Therefore, it is controversial how to balance the optimal extent/number of dissected lymph nodes for radical resection with the lower risk of severe complications. In addition, whether the lymph node dissection strategy should be modified after neoadjuvant therapy needs to be investigated, especially for patients who have a complete response to neoadjuvant therapy. Herein, we summarize the clinical experience on the extent of lymph node dissection in China and worldwide, aiming to provide guidence for the extent of lymph node dissection in esophageal cancer.

ABSTRACT

Pseudoexfoliative glaucoma(PEXG)is an eye disease that seriously endangers vision. It is more invasive than primary open-angle glaucoma(POAG), with more serious damage to the optic nerve, worse prognosis and higher resistance to treatment. Early diagnosis of PEXG can help to treat the disease in time and delay the progress of the disease, so it is important to determine appropriate biomarkers. In recent years, more and more people have begun to study the biomarkers of PEXG, hoping to understand the pathogenesis of the disease, find out the potential early diagnosis and treatment targets of PEXG, and provide some help to the disease through the research of genomics, transcriptomics, proteomics, metabolomics and lipomics markers. This article will review the progress of biomarkers of PEXG in recent years, some biomarkers may provide new ideas for early diagnosis of PEXG in the future.

ABSTRACT

Objective:To explore the disturbance of metal element balance in mice after exposure to radon. Methods:Mice were randomly divided into control group, radon exposure of 30 WLM group, 60 WLM group and 120 WLM groups, with 10 mice in each group. After radon exposure with the cumulative dose, the lung function of mice was detected by a non-invasive pulmonary function testing instrument. Mice blood was taken from eyeballs. The lungs, heart, liver, kidney and spleen were also collected. HE staining was used to observe the pathological changes of lung tissue. Inductively coupled plasma mass spectrometry (ICP-MS) was used to detect the content of metal elements, including essential trace elements in the body: chromium (Cr), molybdenum (Mo), cobalt (Co), selenium (Se), copper (Cu), zinc (Zn), manganese (Mn), nickel (Ni), and potentially toxic elements: arsenic (As), tin (Sn), lead (Pb), aluminum (Al), mercury (Hg), cadmium (Cd), and silver (Ag).Results:Compared with the control group, lung ventilation function of the radon-exposed mice was decreased, alveolar structure was destroyed, and the contents of pulmonary metal elements Cr, Al, Pb, Sn( F=0.34, 0.66, 3.14, 1.16, P<0.05) and essential trace elements Mn, Cr, Zn, and Mo in the blood were decreased( F=0.65, 1.44, 0.97, 2.08, P<0.05), while the elements of Cu, Mo, Se and As in the lungs were increased( F=1.31, 1.26, 0.81, 2.04, P<0.05), and the element contents in other tissues also fluctuated. Conclusions:Inhalation of a certain cumulative dose of radon can reduce the lung ventilation function of mice and induce lung inflammation, as well reduce the content of essential trace elements in the lung and blood so that the content of metal elements in the body fluctuates.

ABSTRACT

Esophageal cancer is a common malignant tumor of the digestive system in China. Currently, surgical resection is the main treatment for localized and resectable esophageal cancer. Minimally invasive treatment of esophageal cancer has the advantages of small trauma, neat incision, less pain, quick postoperative recovery, low postoperative complication incidence and mortality, and the treatment effect is comparable to traditional open surgery. Therefore, minimally invasive surgery for esophageal cancer has gradually become the mainstream choice for esophageal surgery. Various minimally invasive treatment approaches for esophageal cancer have correspon-dingly different indications, advantages and disadvantages. With the continuous development of minimally invasive technology, the shortcomings of various minimally invasive surgical approaches have been continuously overcome, which has brought about the diversification of minimally invasive treatment options. The authors comb the latest research progress at home and abroad, discuss and summarize the current application of minimally invasive techniques in esophageal surgery, hoping to provide references for the clinical minimally invasive treatment of esophageal cancer.

ABSTRACT

ObjectiveTo investigate the protective effect of Liuwei Dihuangwan on neurovascular injury in SAMP8 mice. MethodThe Alzheimer's disease (AD) model with insufficiency of kidney essence was induced in 75 SAMP8 mice aging 6 months. The model mice were divided into model group, positive control group (donepezil hydrochloride, 0.747 mg·kg-1·d-1), and high-, medium-, and low-dose Liuwei Dihuangwan groups (2.700, 1.350, 0.675 g·kg-1·d-1), with 15 mice in each group. Fifteen SAMR1 mice were assigned to a normal control group. All mice were administered continuously for 2 months. The spatial memory of mice was tested by the Morris water maze. Hematoxylin-eosin (HE) staining was used to observe the pathological changes in the hippocampus and cortex of brain tissues. The immunohistochemical method (IHC) was used to detect the deposition of amyloid β-protein (Aβ) and the expression of von Willebrand factor (vWF) and CD34 in the hippocampus and cortex of brain tissues. Electron microscopy was used to observe the ultrastructural changes in cerebral microvessels. Western blot was used to detect the protein expression levels of the receptor of advanced glycation endproduct (RAGE), low-density lipoprotein receptor-related protein 1 (LRP1), vascular endothelial growth factor A (VEGF-A), and P-selection in the hippocampus and cortex of brain tissues. ResultCompared with the normal control group, the model group showed prolonged escape latency and swimming distance (P<0.01), increased number of glial cells, decreased number of nerve cells, blurred tight junctions or enlarged gap of the brain microvascular endothelial cells, severely injured membrane structure, swollen mitochondria of endothelial cells, ruptured membrane, massive dissolution in cristae, increased protein expression of Aβ and vWF in the hippocampus and cortex (P<0.01), reduced protein expression of CD34 (P<0.05), elevated protein expression of RAGE and P-selection in the cortex (P<0.01), and decreased protein expression level of LRP1 and VEGF-A (P<0.01). Compared with the model group, the Liuwei Dihuangwan groups showed shortened escape latency and swimming distance (P<0.05), reduced number of glial cells in the cortex and hippocampus, increased number of microvessels in the cortex, clear double-layer membrane structure in tight junctions between the microvascular endothelial cells, increased number of mitochondria with intact membrane and recovered mitochondrial cristae, decreased protein expression of Aβ, vWF, RAGE, and P-selection in the hippocampus and cortex (P<0.05), and increased protein expression of CD34, LRP1, and VEGF-A (P<0.05). ConclusionLiuwei Dihuangwan can regulate Aβ metabolism through the RAGE/LRP1 receptor system and promote cerebral microvascular angiogenesis by inhibiting vWF expression and increasing VEGF-A and CD34, thereby improving cerebral microvascular injury in SAMP8 mice.

ABSTRACT

ObjectiveTo investigate the effect of Liuwei Dihuangwan on memory function of senescence-accelerated mouse prone 8 (SAMP8) mice by regulating autophagy through the phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt)/forkhead box O3a (FoxO3a) pathway. MethodSix male senescence-accelerated mouse resistant 1 (SAMR1) mice of SPF grade aging 6 months were assigned to a normal group, and 24 male SAMP8 mice of SPF grade aging 6 months were randomly divided into a model group, a donepezil group (0.747 mg·kg-1), and high- and low-dose Liuwei Dihuangwan groups (2.700 and 1.350 g·kg-1), with 6 mice in each group. The mice were treated with drugs by gavage for 2 months. Morris water maze was used to detect the learning and memory abilities of mice in each group. Nissl staining was used to observe the neurons in the cortex and hippocampus. The positive expression of microtubule-associated protein 1 light chain 3B (LC3B) in the cortex and hippocampus was detected by immunohistochemistry (IHC). Western blot was used to detect the protein expression of the mammalian ortholog of yeast ATG6 (Beclin-1), B cell lymphoma-2 (Bcl-2), autophagy-related gene 5 (ATG5), cysteinyl aspartate-specific protease 3 (Caspase-3), Caspase-9, Akt, p-Akt, FoxO3a, and p-FoxO3a. ResultCompared with the normal group, the model group showed prolonged escape latency (P<0.05,P<0.01), reduced number of platform crossings and the residence time in the target quadrant (P<0.01), decreased neurons with reduced volume and dispersed distribution in the cortex and hippocampus, increased positive expression of LC3B (P<0.01), elevated expression of Beclin-1 and ATG5 in the cortex (P<0.01), declined Bcl-2 expression (P<0.01), up-regulated Caspase-3 and Caspase-9 expression (P<0.01), and decreased expression levels of p-Akt/Akt and p-FoxO3a/FoxO3a (P<0.01). Compared with the model group, the donepezil group and the Liuwei Dihuangwan groups showed shortened 3 d escape latency (P<0.05,P<0.01), increased number of platform crossings (P<0.01), and prolonged residence time in the target quadrant (P<0.01). In the donepezil group, the number of neurons in the cortex and hippocampus was increased. In the Liuwei Dihuangwan groups, the number of neurons and Nissl bodies increased with denser distribution and lower degree of cell damage. The positive expression of LC3B in the cortex and hippocampus was decreased in the donepezil group and Liuwei Dihuangwan groups (P<0.01). The expression of Beclin-1 was decreased in the Liuwei Dihuangwan groups (P<0.01). The expression of ATG5 was decreased in the donepezil group and the low-dose Liuwei Dihuangwan group (P<0.01). The donepezil group and the Liuwei Dihuangwan groups showed the increased expression level of Bcl-2 in the cortex (P<0.01), decreased expression level of Caspase-3 (P<0.01), reduced expression level of Caspase-9 (P<0.05,P<0.01), and elevated expression levels of p-Akt/Akt and p-FoxO3a/FoxO3a (P<0.01). ConclusionLiuwei Dihuangwan can effectively improve the learning and memory abilities of the SAMP8 mice and protect neurons. Its mechanism may be related to the regulation of the PI3K/Akt/FoxO3a signaling pathway, down-regulation of the expression of ATG5, Beclin-1, and LC3B, and the inhibition of apoptosis.

ABSTRACT

Pathogenic bacterial infection is one of the main clinical symptoms. Antibiotics are widely used in clinical practice to inhibit or kill the bacteria, fungi and other pathogenic microorganisms. However, with the massive use of antibiotics, drug-resistant strains continue to appear that make the antibacterial situation is becoming increasingly severe. Due to the advantages of multiple targets, multiple pathways and multiple components, traditional Chinese medicine (TCM) have gradually attracted more attention and were used in antibacterial treatment. However, some antimicrobial TCM have problems such as low solubility, poor stability, and low bioavailability. Improving and enhancing the antibacterial activity of TCM through preparation technology is one of the effective solutions. Based on this, two aspects of unilateral antibacterial TCM preparation technology and combination antibacterial preparation technology are introduced, including inclusion technology, nanotechnology, electrospinning, 3D printing and others. Distinctive features and specific application effects of these preparation technologies are explained firstly, and then their advantages and disadvantages are compared and analyzed. The review can be a useful reference for improving the antibacterial activity of TCM.

ABSTRACT

Objective:To investigate the application of therapeutic grade ionization chamber to rapid measurement of short pulsed and high-dose-rate X-ray.Methods:The half-value layer of pulsed X-ray caused by an electron accelerator was measured by interpolation method and its equivalent energy was estimated. The cumulative doses from a certain amount of pulsed radiation at different distances in the same direction around the equipment were compared using the therapeutic grade ionization chamber and thermoluminescence measurement method . The relationship between the measurement result by using ionization chamber dosimeter and the distances away from source was analyzed. The cumulative doses from a certain amount of pulsed radiation at the same location at different frequencies were compared. Results:In working condition, 100 pulses of radiation were received accumulatively at 1 to 12 meters away from the outer wall of the equipment. The range of air Kerma was 0.08-9.65 mGy measured by using thermoluminescence dometers and 0.08 - 9.85 mGy using the ionization chamber dosimeters, respectively. The difference between both is within 10%. At different frequencies (1-10 Hz), there was no significant difference in X-ray air Kerma from 100 pulses measured by ionization chamber dosimeter at 2 m away from the front of the equipment ( P>0.05). Conclusions:The therapeutic grade ionization chamber dosimeter can be used for the rapid measurement of short pulsed X-ray radiation dose in the range of dose rates and pulse frequencies involved in the experimental accelerator device.

ABSTRACT

@#Objective     To analyze the perioperative outcomes of uniportal thoracoscopic lobectomy compared with three-port thoracoscopic lobectomy. Methods     Data were extracted from the Western China Lung Cancer Database, a prospectively maintained database at the Department of Thoracic Surgery, West China Hospital, Sichuan University. Perioperative outcomes of the patients who underwent uniportal or three-port thoracoscopic lobectomy for lung cancer during January 2014 through April 2021 were analyzed by using propensity score matching. Altogether 5 817 lung cancer patients were enrolled who underwent thoracoscopic lobectomy (uniportal: 530 patients; three-port: 5 287 patients). After matching, 529 patients of uniportal and 1 583 patients of three-port were included. There were 529 patients with 320 males and 209 females at median age of 58 (51, 65) years in the uniportal group and 1 583 patients with 915 males and 668 females at median age of 58 (51, 65) years in the three-port group. Results     Uniportal thoracoscopic lobectomy was associated with less intraoperative blood loss (20 mL vs. 30 mL, P<0.001), longer operative time (115 min vs. 105 min, P<0.001) than three-port thoracoscopic lobectomy. No significant difference was found between the two groups regarding the number of lymph node dissected, rate of conversion to thoracotomy, incidence of postoperative complication, postoperative pain score within 3 postoperative days, length of hospital stay, or hospitalization expenses. Conclusion     Uniportal video-assisted thoracoscopic lobectomy is safe and effective, and the overall perioperative outcomes are comparable between uniportal and three-port strategies, although the two groups show differences in intraoperative blood loss.

ABSTRACT

@#Objective     To identify the risk factors for postoperative recurrence of peripheral solid small-nodule lung cancer (PSSNLC) (T≤2 cm), and to explore the effects of surgery types on prognosis. Methods     We extracted data from Western China Lung Cancer Database (WCLCD), a prospectively maintained database at the Department of Thoracic Surgery, West China Hospital, Sichuan University, and Surveillance, Epidemiology, and End Results (SEER)  database for peripheral solid small-nodule lung cancer patients (T≤2 cm N0M0, stageⅠ) who underwent surgery between 2005 and 2016. We used univariable and multivariable logistic regression to analyze risk factors for recurrence of PSSNLC. We applied propensity-score matching to compare the long-term results of segmentectomy and lobectomy, as well as the survival of patients from WCLCD and SEER. We finally included 4 800 patients with PSSNLC (T≤2 cm N0M0)(WCLCD: SEER=354∶4 446). We matched 103 segmentectomies and 350 lobectomies in T≤1 cm, and 280 segmentectomies and 1 067 lobectomies in 1 cm<T≤2 cm at a ratio of 1∶4 according to the propensity scores. Results     The results of multivariable logistic regression showed that older age (HR=1.04, 95%CI 1.03-1.05, P<0.001), male (HR=1.60, 95%CI 1.37-1.88, P<0.001), squamous carcinoma (HR=1.65, 95%CI 1.40-1.95, P<0.001), lymph node removed (HR=0.97, 95%CI 0.96-0.99, P<0.001) were risk factors for recurrence after surgery. We found that segmentectomy and lobectomy could reach similar survival in PSSNLC patient with either T≤1 cm or 1 cm<T≤2 cm. Analyses of 1 441 patients with PSSNLC after matching (WCLCD∶SEER=325∶1 116) indicated that the 5-year overall survival rate of the patients in WCLCD was better than that in SEER database ( 89.8% vs. 77.1%, P<0.001). Conclusion     Older age, male, squamous carcinoma, and lymph node removed are the risk factors for recurrence of PSSNLC. Segmentectomy shows similar survival in PSSNLC patient with either T≤1 cm or 1 cm<T≤2 cm N0M0. The patients in the WCLCD shows better survival compared with of the patients in the SEER database.

ABSTRACT

Objective:To investigate the relationship between the dose of preoperative neoadjuvant radiotherapy and the pathologic complete response (pCR) rate in patients with locally advanced squamous cell esophageal cancer (ESCC).Methods:Clinical data of 116 patients with ESCC who received neoadjuvant chemoradiotherapy followed by esophagectomy in our cancer center from July 2017 to December 2019 were retrospectively analyzed. The radiation doses were divided into 2 ranges based on Grays (Gy) received: 40-45 Gy and 45 Gy or more. Results:The overall pCR rate was 38. 8%(45/116). pCR was observed in 35 out of 80(44%) patients treated with 40-45 Gy and 10 of 36(28%) patients treated with 45 Gy or more. The pCR rate did not significantly differ between two groups [(40-45 Gy) vs.( ≥ 45 Gy), P=0.105)]. Conclusions:Preoperative neoadjuvant radiotherapy with a higher dose (≥ 45 Gy) fails to increase the pCR rate in patients with locally advanced ESCC. Prospective randomized trials are required to determine the optimal dose of preoperative adjuvant radiotherapy.

Objective:To study the correlation between autoantibodies against AT1 receptors (AT1-AAs) and risk of lupus nephritis (LN).Methods:Thirty-six normal subjects and 32 patients with LN were selected. The AT1-AAs in sera of donors were detected by enzyme-linked immuno sorbent assay (ELISA). The level of AT1-AAs, C-reactive protein (CRP), serum creatinine, lipids and serum glucose in different groups were observed. We tested the serum cystatin c, erythrocyte sedimentation rate (ESR), complement C3, C4, antinuclear antibodies (ANA) and anti-dsDNA antibodies in LN patients. The structures of the glomeruli were stained by renal biopsy hematoxylin-eosin staining (HE) and schiff periodic acid shiff (PAS) . Statistical analysis was performed using independent sample t-test, Chi-square test, Spearman correlation analysis and multiple linear regression. Results:Serum creatinine and CRP were significantly higher in the LN group [(103±37) μmol/L, (21±8) mg/L, respectively] than in the control group [(72±22) μmol/L, (7±3) mg/L], all P<0.01. The average A value of AT1-AA was higher in LN patients (0.33±0.04) than in controls (0.25±0.06), P<0.01. Multiple linear regression analysis showed that the expression of AT1-AA was positively correlated with the amount of CRP ( Beta=0.364, P=0.037) and urine protein ( Beta=0.497, P=0.004), and was negatively correlated with the level of complement C3 ( Beta=-0.276, P=0.05). Pathological results showed that mesangial hyperplasia and capillary endothelial proliferation and glomerulosclerosis were more obvious in the AT1-AA positive group than the AT1-AA negative group. Conclusion:The expression of AT1-AA is significantly increased in patients with LN and correlated with CRP, urinary protein and complement C3, indicating that the AT1-AA may be involved in the pathogenesis and the progression of LN.

ABSTRACT

@#Objective    To compare the perioperative outcomes of subxiphoid robot-assisted extended thymectomy (SRAET) and video-assisted thoracoscopic extended thymectomy (VATET) for myasthenia gravis complicated with thymoma. Methods    Retrospective analysis of 61 patients with myasthenia gravis combined with thymoma who were admitted to the Department of Thoracic Surgery, West China Hospital, Sichuan University from January 2017 to June 2019 was performed. All patients underwent extended thymectomy, and the patients were divided into a SRAET group and a VATET group. There were 26 patients in the SRAET group, including 11 males and 15 females, with an average age of 42.20±13.20 years. There were 35 patients in the VATET group, including 14 males and 21 females, with an average age of 45.00±13.00 years. The perioperative outcomes of the two groups including gender, age, operation time, intraoperative blood loss, conversion rate, postoperative drainage, tube removal time, drainage volume, visual analogue scale, hospital stay and postoperative complications were compared. Results    There was no conversion to thoracotomy, death or myasthenia crisis in both groups. The operation time (111.42±28.60 min vs. 103.71±26.20 min, P=0.845), intraoperative blood loss (32.31±23.84 mL vs. 63.57±132.22 mL, P=0.239), visual analogue scale at postoperative 24 h (2.46±0.76 vs. 2.40±0.74, P=0.751) and postoperative 48 h (2.12±0.77 vs. 2.26±0.56, P=0.407), complication rate (3.8%vs. 2.9%, P=0.675), drainage volume (206.85±130.09 mL vs. 276.86±173.46 mL, P=0.089) and hospital stay (5.81±2.52 d vs. 5.29±2.17 d, P=0.642) were not significantly different between the two groups. The visual analogue scale of the SRAET group at postoperative 72 h (1.12±0.65 vs. 1.86±0.91, P=0.001) was significantly lower than that of the VATET group. Conclusion    SRAET is a safe and feasible method with less postoperative short-term pain, which is an alternative surgical treatment for myasthenia gravis complicated with thymoma.

ABSTRACT

There is no doubt that the colonial period was a critical time for the establishment and expansion of modern Western medicine in Korea. However, did this act as a catalyst for the overall decline of traditional Korean medicine? While previous studies mainly focus on research based on the concept of Uisaeng (traditional Korean medicine doctor) and the medical policies implemented by the Japanese Government-General of Korea, this paper begins with the Korean herbal medicine industry, and comprehensively investigates the distribution and consumption of Korean herbal medicines during the colonial period from three perspectives: the policies for Korean medicine merchants implemented by the Japanese Government-General of Korea, changes in the Korean herbal medicine industry, and consumption of Korean herbal medicines in the Korean society. The colonial authorities’ intention was to foster the advancement of Western medicine and phase out traditional Korean medicine. However, they merely imposed limitations on Uisaengs’ operations—this policy loophole objectively left a window for Korean medicine merchants. Moreover, against the backdrop of the growing popularity of Western medicine and restrictions on the development of traditional Korean medicine by colonial authorities, the Korean herbal medicine industry, as one of the few “national industries” dominated by and serving Koreans, showed its tenacious vitality during that time. Korean medicine merchants responded to market changes with ease. They built different drugstores, such as traditional herbal stores mainly selling traditional Korean medicines, hybrid drugstores that simultaneously dealt with the manufacture and sale of patent medicines, and ginseng drugstores that specialized in the ginseng business. This classification promoted the commercialization of traditional Korean herbal medicine.Another crucial condition for the vitality of the Korean herbal medicine industry is Koreans’ preference for traditional Korean medicine. It is an indisputable fact that Western medicine gradually became popular and was recognized by the common man during the colonial period; nonetheless, Eastern medicine and Western medicine were not playing a zero-sum game. Through comprehensive macro and micro analysis, this paper demonstrates that, during the colonial period, when old and new ideas interacted, most Koreans, including upper-class elites and intellectuals who were open-minded about emerging concepts and options and had ample opportunities to avail western medical treatment, preferred traditional Korean medicine. Using Korean herbal medicines for illnesses remained the primary choice, While Western medicine assumed the role of a supplement to traditional treatment. This paper argues that the first reason for this phenomenon is the inertia of tradition, and the second is that Western medicine was not necessarily more effective than Korean herbal medicine at that time.Specifically, it can be considered that, during the colonial period, the growing popularity of Western medicine failed to bring about a radical change in Koreans’ regular medical interventions. Simultaneously, the Korean herbal medicine industry, one of the pillars offering medical support to the common man, adapted suitably while relying on the inertia of its own tradition. The industry’s vitality and dynamism during the colonial period certainly underscore the need to amend the one-sided narrative of medical modernization vis-à-vis Western medicine.
